Sports Physical Packet Checklist

• Pre-Participation Physical Evaluation Form - MUST BE DATED AFTER JUNE 1
• Athletic Consent Waiver Form (completed by parent/guardian)
• Vital Health Record (completed by parent/guardian)
• Concussion Information Form (completed by parent/guardian)

IMPORTANT!

All required health forms must be completed and submitted through Magnus Health. Forms can be completed directly within the Magnus Health portal, and the physician-completed Physical Examination Form should be uploaded there as well. Please follow the instructions below to complete and submit all required documentation. Paper forms will not be accepted.

Magnus Health

The Magnus Health app is available for download from the Apple App Store and Google Play Store.

Before using the app for the first time, you must access Magnus Health through the Parent Portal to activate your account. Once logged in, you can use the app to view your student's health records, upload required documents such as physicals and immunization records, and complete and electronically sign required forms.
